Updated Provisional Assisted Living License Application Now Available

The Minnesota Department of Health (MDH) Health Regulation Division has posted an updated provisional assisted living license application for providers interested in becoming an assisted living facility. The revised form has added instructional information and links to helpful statutory references.
The form leads applicants to indicate if they are applying to license a new facility or existing facility due to a change of ownership (CHOW).
Because licensing applies to the services and the building, the license is tied to the physical address. Therefore, a provider also must apply for a new license if they move their assisted living.
All new provisional assisted living license applications must be submitted on the new form. ALL applications previously submitted and in progress may continue on the old version of the application.
The new form includes updated information clarifying the application requirements and additional fields to help applicants complete the form accurately.
