UDALSA Tip Sheet Developed by OOLTC

The Uniform Disclosure of Assisted Living Services and Amenities (UDALSA) is an important communication tool for providers to inform prospective residents and their families about what their Assisted Living Facility provides.
The Office of Ombudsman for Long-term Care (OOLTC) recently developed a tip sheet to assist consumers in locating the UDALSA for facilities on the Minnesotahelp.info website.
Resource to Help Prospective Residents
The UDALSA for a facility has been linked to the facility information on Minnesotahelp.info. By linking these UDALSA’s in this information platform, a prospective resident/representative can view the UDALSA for any facility they are interested in to see if the facility can meet their needs. The tip sheet also provides sample questions to assist a prospective resident in further understanding the offerings and limitations of any given facility.
Provider Requirements
Assisted Living Facilities are required to submit a completed checklist, on the State template, with a license application or renewal. Whenever a facility changes services offered under the assisted living facility contract, the facility must submit an updated checklist within 30 calendar days of the change in services. Providers may want to check that the UDALSA on Minnesotahelp.info is the most current version.
In addition to providing the UDALSA checklist to each prospective resident and each prospective resident's representatives who request information about the facility, providers are required to provide an oral explanation of the services offered prior to the execution of the assisted living contract. This provides an excellent opportunity to discuss anticipated long-term needs and how those needs can be addressed.
Requirements for the UDALSA are found in 144G.40 and the AL Rule 4659.0090.
