Second Extension Given by DHS for Survey on Costs and Wages to be Used for Rate Methodology Evaluation – Please Participate

DHS has further extended the deadline through the end of March for providers to participate in the survey about costs and wages. Assisted Living and Adult Day Members of LeadingAge MN are especially encouraged to participate.
The survey supports the legislatively mandated rate methodology evaluation for several services provided under:
- Elderly Waiver (EW).
- Community Access for Disability Inclusion (CADI) Waiver.
- Brain Injury (BI) Waiver.
- Alternative Care (AC) program.
- Essential Community Supports (ECS) program.
The survey will allow providers to help inform the components and factors of the rate methodology for critical services.
Services included in the survey are:
- Adult day services.
- Chore services.
- Home-delivered meals.
- Homemaker.
- Adult companion services.
- Respite.
- Individual community living supports.
- Adult foster care.
- Customized living services.

Providers that do not have a business tax identification number or use their social security number as a business tax identification number do not need to complete a survey.
