Revised Uniform Disclosure of Assisted Living Services and Amenities (UDALSA) Posted

An updated Uniform Disclosure of Assisted Living Services and Amenities (UDALSA) form is now available from the Minnesota Department of Health (MDH).
The new form has been made easier to use by simplifying the instructions, expanding the comments section, and offering space to explain features unique to your setting. Only one UDALSA form per facility is allowed, a change from initial licensure. Changes were made so that, with only one UDALSA, providers can communicate to prospective residents what services are available and if there are variations between sections of a facility or different buildings on a licensed campus.
Notable changes
Assisted living settings should note the following:
- Facilities are not required to use the updated UDALSA form immediately.
- Facilities will submit the new form with the following license renewal and future renewals.
- Whenever a facility changes the services offered under the assisted living facility contract, the facility must submit an updated UDALSA to the commissioner within 30 calendar days of the change in services at health.assistedliving@state.mn.us.
The new UDALSA form should be provided to each new prospective resident and each prospective resident's representative who request information about the facility as required in Minnesota’s Assisted Living Rules.
You are not required to distribute the new UDALSA form to current residents. However, LeadingAge Minnesota encourages you to provide a copy of any revised forms to all residents/representatives so that they are kept informed of services offered and any limitations to services.
Whenever a facility changes its services under the assisted living contract, it must submit an updated UDALSA to the commissioner within 30 calendar days of the service change.
