MDH Offers One-Time PPE Request Opportunity

The Minnesota Department of Health (MDH) is currently offering a one-time disbursement of personal protective equipment (PPE) to improve infection prevention capacity in assisted living organizations and nursing homes.
Available PPE and request process
This program is first come, first served. Available products include N95 masks, face masks, gowns, gloves, face shields, thermometers, bandages, and some other items. Organizations will receive products at no cost. In order to receive PPE from the state warehouse, use the Warehouse PPE Request Form. If your organization submitted a request previously, you will need to re-submit.
Things to Know
- If you submit a request and do not receive all ordered items, it is likely those items are no longer available.
- MDH will work with organizations when shipping larger orders
- This is for assisted living and nursing homes only; please do not share the Warehouse request form link
- Items cannot be returned to the warehouse
