License Transition Support Opportunity Available from DHS for Certain Providers

Assisted Living licensed providers of customized living services who applied before December 31, 2023, to transition to DHS licensing as a community residential setting may qualify for a license transition support opportunity.
License Transition Details
DHS will distribute one-time payments to Medical Assistance (MA) disability waiver service providers who transitioned or will transition from a licensed CL setting to a licensed community residential setting under a corporate foster care moratorium exception.
For more information about license transition support, refer to Laws 2024, chapter 127, article 46, section 43 and article 53, section 2, subdivision 14.
To be eligible to apply, providers must meet all the following conditions:
- Register in Minnesota’s Statewide Integrated Financial Tool System (SWIFT). For instructions, refer to MMB – SWIFT vendor resources.
- Select a payment method (i.e., paper or electronic) in SWIFT.
- Have received DHS approval for a corporate foster care moratorium exception between July 1, 2022, and Dec. 31, 2023, for each eligible setting for which they request a payment.
- Either have an active community residential setting license or have a pending application to transition from a CL setting to a licensed community residential setting, under the moratorium exception.
Note: Receiving funding does not guarantee community residential setting licensure for organizations with pending license applications. - Be in good standing according to DHS – Licensing information lookup if they have an active community residential setting license.
- Intend to provide services at each eligible setting through Dec. 31, 2027.
Organizations must submit their electronic application by 11:59 p.m. on March 1, 2025. DHS will not consider late or incomplete applications.
