Health Information Exchange Opportunity for Nursing Homes

Audacious Inquiry (Ai), a health information technology company, will present a free webinar on May 25 from 9-10 a.m. to share information with LeadingAge Minnesota nursing home members about a health information exchange opportunity.
In collaboration with the Minnesota Department of Human Services (DHS), Ai is delivering an Encounter Alert Service (MN EAS) to help support interoperability across the care continuum in Minnesota. MN EAS enables providers throughout the state to receive patient encounter alerts for individuals who have been admitted to or discharged or transferred from an EAS-participating hospital, emergency department, long-term care facility, or other provider organization in real-time. The goal of these alerts is to help achieve better outcomes and care transitions for patients by facilitating the electronic exchange of clinical data between providers and enabling real-time care coordination. Currently, roughly 100 nursing homes are connected to this network, as are nearly all hospitals in Minnesota.
There is funding available through June 30 to cover the costs of participation in the MN EAS – specifically, installation and two years of service fees – for nursing homes that are interested. To make members aware of the opportunity, this May 25 program will describe how the health information exchange service works, how it supports automated clinical data exchange and scenarios in which nursing homes might use and benefit from the system.
