Get to Know Me Information Helpful to Staff Caregivers

In this time of the COVID-19 pandemic, there are instances when residents are cared for by new or temporary staff or need to be transferred without family to accompany them. As staff work to provide the best possible care to residents, a brief information sheet about the individual that can be kept in the person’s room or apartment for easy reference can be very helpful.
A hard copy of this document can be easily referenced or transferred in case of a hospitalization, a cohorting situation or evacuation.
If a resident is able to complete the form themselves, it can reassure them that a caregiver will know a bit about themselves should they be too ill to share directly.
For family members, unable to visit during this time, completing this form for a loved one who is unable to complete the form themselves provides them an opportunity to do something to support and care for their family member.
