Funding Available for Resident and Family Peer Support Group Program

The Minnesota Department of Human Services (DHS) is seeking proposals from qualified Minnesota Medicaid-certified nursing facilities interested in implementing a resident and family peer support group program.
The ResoLute Program (Resident Empowered Solutions on LIVING Until the End) embraces the concept of aging with purpose and determination through a conversation-based program that fosters connection and deepens relationships. This program addresses social isolation and loneliness by creating resident and family peer support groups and using guided questions outlined in the ResoLute journals. This program was created by the Empira Consortium, with 24 Minnesota long-term care communities, and was recognized by the Pioneer Network as a recipient of the Commitment to Community Award.
Participating facilities will be awarded grant funds through the state's Civil Monetary Penalties (CMP) Initiative to cover the costs of facilitator education and support, facilitator guides, resident and family journals, and access to activities and tools.
An informational webinar will be held on Wednesday, Oct. 20 at 1 p.m. All applications must be received by midnight on Dec. 1. Nursing facilities interested in participating in this grant opportunity should visit the DHS CMP Initiative website.
