DHS Accepting Proposals for Customized Living Quality Improvement Grants

The Minnesota Department of Human Services (DHS) is seeking proposals for the new Customized Living Quality Improvement Grants, which support provider-initiated projects that improve quality, encourage innovation and advance efficient, high quality service.
Qualified responders must have provided Customized Living services to Elderly Waiver program participants between July 1, 2018 and June 30, 2019. Preference will be given to providers that serve at least 75% EW participants. The contract period for grant recipients is Jan. 1, 2020 - June 30, 2021.
The DHS Aging and Adult Services Division released a report of quality measurement in assisted living in 2019 as part of its efforts to create an Assisted Living Report Card. The report identified nine domains of quality that providers should work from in formulating their grant proposal:
- Resident quality of life
- Resident and family satisfaction
- Safety
- Resident health outcomes
- Staff
- Physical and social environment
- Service availability
- Core values and philosophy
- Care services and integration
Questions? Questions must be submitted in writing by 4 p.m. on Oct. 14. Email questions to dhs.aasd.hcbs@state.mn.us. Questions will be addressed and posted to the RFP website no later than Oct. 21.
Proposal Submission: Proposals must be submitted online by 4 p.m. on Oct. 28. The online application process requires the use of templates provided. Consider logging into the application portal in advance of the deadline to review the templates and the submission portal.
