The Minnesota Department of Human Services (DHS), through the Nursing Facility Rates and Policy (NFRP) Division and the State Civil Monetary Penalties (CMP) Initiative, is seeking proposals from qualified Minnesota Medicaid-Certified Nursing Facilities that are interested in implementing a Dementia Live Training Program.
Dementia Live is an experiential simulation dementia care training program for direct care and support staff that seeks to enhance the quality of life and/or quality of care for nursing facility residents with dementia. AGE-u-cate Training Institute will provide the training.
Qualified Minnesota nursing facilities interested in participating in this grant opportunity are asked to read the Request for Proposals and submit a CMP application form to the DHS. Applications must be received by midnight on July 1.
An informational webinar will be held on Thursday, April 22, at 1 p.m. To register for the webinar, to download the application, for technical assistance with your application, or other questions regarding this funding opportunity or the MN CMP Program, please visit DHS’s Civil Monetary Penalty webpage and select the link for “Dementia Live Training Grants.”
