Customized Living Quality Improvement Grant RFP Announced

A grant opportunity was announced this week for providers of Customized Living Services through BI, CADI and EW. The goal of this grant is to help providers improve overall quality of life for people who receive these services.
Customized Living Services are those provided in Assisted Living licensed facilities or settings exempt from AL licensure.
Funding and Eligibility
This is the fourth round of this grant which was established in 2019. DHS expects to award up to $2 million in grants to successful responders from March 2025 to June 2026. One-time grants ranging from $25,000 to $250,000 will be available.
To be eligible for this grant, your setting(s) must meet the following requirements.
- The Responder must be an enrolled provider with DHS to provide Customized Living Services under BI, CADI or EW.
- At least 75 percent of the residents served by the provider must be waiver participants.
- For providers of Customized Living Services under the BI or CADI waivers, the waiver participants must reside at multiple locations each with six or more residents.
Qualified responders will need to explain how their proposal can positively affect the following domains of quality:
- Person’s quality of life
- Person and their family’s satisfaction
- Safety
- Person’s health outcomes
- Staff
- Physical and social environment
- Service availability
- Core values and philosophy
- Care services and integration

Process to submit a proposal
To submit a proposal in response to this RFP, responders must use DHS and Minnesota Board on Aging – Online grant portal.
Responders cannot submit proposals through mail, fax or email.

Deadline for submission
Responders must submit their proposal through the online application portal by 4 p.m. on Friday, Dec. 13, 2024. DHS will not accept late submissions.
