Connected Communities Grant Webinar

Do you ever wish you could build a better system to support the range of health and wellbeing needs of adults as they age? If you do, and if you missed our Connected Communities grant webinar last week, you can now watch it here.
We assembled a panel of speakers, including those leading the Connected Communities projects at Perham Health and Knute Nelson/Walker Methodist to share their insight on the progress they have made over the last several years and what this could look like in your community.
What do the initial pilot sites think of Connected Communities?
“We really saw opportunity in our community to do more for people. And as a provider it can be really frustrating because we can see the wave coming at us, but we don’t necessarily have the ability to make changes. This is that opportunity to make changes and have a chance to transform what our communities look like and build a better future . . . A new Connected Communities grant opportunity is available to LeadingAge Minnesota members outside the seven-county Twin Cities metro area.
