Check your MNITS Mailbox if you are a HCBS Provider: Revalidation in Process

The revalidation process has begun for providers enrolled with DHS as a Home and Community Based Service (HCBS) provider which includes waiver services for Adult Day Services, Assisted Living (Customized Living), as well as Home Health Agencies.
All providers are required to complete revalidation at least one time every five years. Providers will receive a notice when it is time for revalidation and can expect to receive an initial revalidation notice as soon as 3 ½ years after the most recent revalidation or enrollment date.
Check your MN–ITS PRVLTR file regularly for important notices, including letters indicating you are due for revalidation. If you have not set up a MN–ITS mailbox, MHCP will send your notice of revalidation to the credentialing address recorded on your enrollment record.
- You have 30 days from the date on the notice to upload your required documents in the Minnesota Provider Screening and Enrollment (MPSE) portal or fax in all required documents.
- If MHCP does not receive all required information, they will send you a request for more information (RFMI) notice asking for the remaining required information to complete revalidation. This information is due 30 days from the date on the RFMI notice.
- If they do not receive all required information 30 days after the RFMI notice date, they will send you a 60-day notice of termination including provider appeal rights and options for reenrolling after termination.
If you are enrolled as more than one provider type, you will receive a separate notice for each type. If you are consolidated and have more than one file for a provider type, you will receive one notice with instructions to send the required information for each file. If you are subject to a fee, you must pay the fee at the time you submit your revalidation documents.
If you are assigned a high or moderate risk level, you are subject to a site visit. The post-enrollment site visit may occur any time within the five years before your next revalidation. You can continue receiving payments from MHCP while the site visit is pending.
The first round of HCBS providers undergoing revalidation includes 174 providers. If your organization was selected for revalidation, you would have received a revalidation notice either in your MN-ITS mailbox, or to your credentialing address on file via mail. Any provider who was sent a notice and who has not submitted their revalidation documents at this time will be receiving their second notice next week.
