Care Centers: New Reports on Hospitalization Rates and Community Discharge

Detailed trend reports for care centers on two new quality measures – hospitalization rates and rate of discharge back to the community – are now available from the Minnesota Department of Human Services (DHS).
Care centers can access these reports via the Nursing Facility Provider Portal. DHS also provided a set of slides summarizing the measures and showing two years of aggregate results. The new measures will eventually be included on the Minnesota Nursing Home Report Card.
Members can use their results to benchmark against other providers statewide and to determine if there are areas where improvement is needed. DHS is also planning to include these measures as options that providers can use to earn incentive payments under the state’s quality programs including PIPP and QIIP.
