Assisted Living Survey Data Updates

MDH recently shared data on surveys of Assisted Living facilities that had been conducted through February 2022. Since the initiation of Assisted Living Licensure, 126 surveys of conversion licensees were completed.
An additional 11 initial surveys and 23 surveys due to change of ownership were also conducted. The average number of correction orders received were 16.361, with an additional 4.318 issued on follow-up surveys.
There are 4 levels of severity for correction orders. Only 18 facilities received level 3 orders with no facility cited at a level 4, the highest level of severity. 16 facilities (from 8 organizations) received conditions on their licenses because of survey findings.
63 fines were issued with 46 fines at a level 2 for $500 each, and 17 fines at a level 3 for $3000 each. Most cited areas include:
- Fire Protection and Physical Environment
- Disaster Planning and Emergency Preparation
- TB Prevention and Control
- Minimum Requirements (144G.41) including:
- Gaps in infection control program (Subd. 3)
- Minnesota Food Code - failure to meet all requirements of the Code (Subd. 1)
- Resident grievances, reporting maltreatment (Subd. 7)
- No proof of staffing logic/metrics, slow response time to call lights, staff sleeping (Subd. 1)
Survey results can be reviewed on the MDH website with searches available by dates, facility, or location.
