Assisted Living Licensure Closure Form Now Available

The Minnesota Department of Health has added an Assisted Living Licensure Closure Form to their Forms and Self-Audit Tools page under the “notifications” heading. The form provides guidance for the closure process.
If an assisted living facility elects to close voluntarily, the facility must notify MDH in writing by submitting the Assisted Living Licensure Closure Form along with a proposed closure plan. Proposed closure plans are subject to the commissioner's review and approval. The facility may not take any action to close the residence before the commissioner approves of the plan.
