Assisted Living License Renewals Open in One Week

The Minnesota Department of Health (MDH) will be sending you renewal information on or before May 1 for your assisted living license renewal for conversion licenses. MDH developed materials to help you prepare your assisted living license renewal application on the MDH website.

MDH recommends you learn, prepare and apply early for your assisted living license renewal starting May 1. You can:
- Learn about the renewal process by attending a webinar or reviewing renewal materials at the Assisted Living Licensure website.
- Prepare for the renewal application by organizing the documentation needed for renewals.
- Apply early for your assisted living license renewal before the June 1 deadline. Renewal information will be sent to providers on or before May 1.
Renewal timeline
The timeline for assisted living renewals is as follows:
- Assisted Living Licensure renewals begin May 1.
- Starting July 1 grace period ends, and fines start ($200).
- After Aug. 1, providers operating without a license will be fined $250 per day.
