Assisted Living Facilities Licensed

The Minnesota Department of Health has issued over 1,700 assisted living facility or assisted living facility with dementia care licenses as of Friday afternoon. For the remaining 286 conversion applicants who are waiting for background checks or who have final documents to submit, MDH has issued a conditional license.
The license numbers have been posted on the provider spreadsheet, and licenses were mailed on Monday. MDH will continue to work through these remaining applications.
The conditional licenses are effective Aug. 1 and will require continued actions to remove the condition from the license. Details regarding the specific condition, steps to follow to lift the condition from the license, and the time allowed to do so will be sent to the provider by MDH.
