As ALL Conversion Application Closes, Providers Move to Provisional Application

As of Wednesday, June 2, the assisted living license conversion application has been frozen, and the deadline to apply has passed. However, if you did not submit your application, you can still move ahead with a provisional application to comply with licensure requirements by Aug. 1.
For providers who submitted applications with missing information or documents, the Minnesota Department of Health (MDH) will be re-open those applications for correction and completion.
By the morning of June 1, MDH had received nearly 2000 conversion applications and had pre-reviewed over 1200 of them. About 700 providers have already paid and moved through to the final review. However, MDH is awaiting payment from almost 300 providers. This is a critical step to keep the application process moving.
Providers who have submitted applications and received a notice to pay need to pay to move to the next stage of the approval. Please check your email, including email spam, for this request from MDH. Please note that there is a service fee when you use a credit card to pay for your license fee. However, if you use a bank account to pay, no fee will be charged.
