Apply Now: Live Well at Home Grants

The Minnesota Department of Human Services (DHS) has published the Request for Proposals (RFP) for the next round of Live Well at Home Grants. These grants help communities meet the challenges of our fast-growing aging population and pressures on Minnesota's long-term care support system.
The complete Request for Proposals, with directions and online application, are available on the DHS Live Well at Home Grants website. Final proposals are due by 4 p.m. on June 29.

Metropolitan Area Agency on Aging provides technical assistance on project needs and design, outcomes, sustainability, and budget, and can review draft proposals as time permits. Applicants are strongly encouraged to Jake Chernugal and Diane Graham-Raff as soon as possible to receive consultation.
